Define what happens when the model is unsure, when tools fail, and when a human rejects a suggestion. Silent failure is worse than a visible queue. Escalation paths keep operations controllable as volume grows.

- Time from suggestion to decision
- Exception volume by reason code
- Incidents caused by skipped review
